dsh-stepwise-distill：固化 DSH 會話歷史

`dsh-stepwise-distill` 是 DSH 中用於解決長對話場景下歷史記錄膨脹問題的插件。它通過分步固化會話歷史，將工具結果重寫爲模型實際保留的事實，防止重複發送已完成內容，從而提升上下文密度。安裝命令爲 `dsh plugin --profile <name> add dsh-stepwise-distill`。該插件默認啓用摘要功能（`stepSummary` 和 `turnSummary`），可通過 `/distill` 端點檢查狀態。使用時需注意其會增加約兩倍的 token 和時間成本，並需要 Node.js >= 20 環境。
