dsh-subagent-model-notes：爲 Subagent 路由補充逐模型說明

DSH 官方 Subagent 設置只提供 provider/model 白名單與 list_subagent_models，缺少逐模型選型說明。社區插件 dsh-subagent-model-notes（097794，工作流）在 Subagent 設置旁增加 Subagent model notes 頁，保存 volatile notes，並在 system-prompt/assemble 時把當前 Session 可選路由的說明寫入委派方 system prompt。安裝爲本地 profile bundle（Plugins 頁或 install_bundle），需 DSH 0.2.1-rc.x 與插件系統；說明僅爲指引，白名單仍由官方 Subagent enforce。
