dsh-subagent-model-notes：为 Subagent 路由补充逐模型说明

DSH 官方 Subagent 设置只提供 provider/model 白名单与 list_subagent_models，缺少逐模型选型说明。社区插件 dsh-subagent-model-notes（097794，工作流）在 Subagent 设置旁增加 Subagent model notes 页，保存 volatile notes，并在 system-prompt/assemble 时把当前 Session 可选路由的说明写入委派方 system prompt。安装为本地 profile bundle（Plugins 页或 install_bundle），需 DSH 0.2.1-rc.x 与插件系统；说明仅为指引，白名单仍由官方 Subagent enforce。
