dsh-stepwise-distill：固化 DSH 会话历史

`dsh-stepwise-distill` 是 DSH 中用于解决长对话场景下历史记录膨胀问题的插件。它通过分步固化会话历史，将工具结果重写为模型实际保留的事实，防止重复发送已完成内容，从而提升上下文密度。安装命令为 `dsh plugin --profile <name> add dsh-stepwise-distill`。该插件默认启用摘要功能（`stepSummary` 和 `turnSummary`），可通过 `/distill` 端点检查状态。使用时需注意其会增加约两倍的 token 和时间成本，并需要 Node.js >= 20 环境。
