dsh-restart-planner：把 DSH 宿主重啓變成 agent 可調用的工作流

dsh-restart-planner 是 7gzgn 維護的 DSH 工作流插件，通過 self_restart 工具把宿主重啓拆成排程、交接與獨立 helper 執行：先結束當前輪次與日誌落盤，再停掉並原地拉起 DSH，結果寫入 ~/.dsh/dsh-restart-planner/last.json。支持 dryRun、cancel、用戶 confirm 選時機，v0.4.0 起可經 sessionController.prompt 向原會話排隊投遞繼續提示。零依賴、純本地；Windows 桌面端與 dsh web 有實測說明。安裝命令以 SkillHub 與 GitHub 爲準；插件以宿主進程權限運行，使用前請審閱源碼與 MIT 許可。
