dsh-plugin-log-bridge：把 Harness 運行時告警與錯誤寫到 stderr

DSH 默認在內存環形緩衝保存日誌且不對外導出，啓動 ready 之後的失敗往往只留下「進程掛了」而無 Harness 側原因，warn 還可能被默認級別濾出緩衝。dsh-plugin-log-bridge 由 desanv01 維護，無 UI、無配置，向 stderr 輸出帶 [harness-log] 前綴的 error/warn，回放加載前緩衝的 error，並訂閱 api-session/error；含限速與 boot 安全包裹。安裝示例：dsh plugin --profile web add dsh-plugin-log-bridge。適用需由父進程捕獲 stderr 的部署場景；注意 stderr 可能含敏感日誌，安裝前請審閱 MIT 源碼。
