@gw/dsh-tools-manager：按 Agent 预设禁用工具

@gw/dsh-tools-manager 是 DSH 社区 admin-security 类插件，按 Agent 预设 id 维护工具禁用黑名单，无白名单。通过 ctx.tools.restrict 与 guard 双层拦截，并提供设置页「工具管理」与 rules.json 持久化。安装需将包放入 profile 的 node_modules/@gw 并在 cordis.patch.yml 注册，适用于按模式关闭 bash 等敏感工具的场景。MIT 许可，要求 DSH >=0.2.1-rc.2 <0.3.0。
