dsh-auto-observe：让 bash 查过的文件也能通过「先读后改」校验

DSH 的 fs 观察策略只认 read/write/edit 等通道，bash 用 sed、awk、cat 看过文件不会登记，智能体常因此收到 file has not been read 拒改。dsh-auto-observe 在 edit/write intent 前补发 fs/observed，用 stat 记录真实版本，且不削弱 FS_STALE_VERSION 与 edit 逐字匹配。安装用仓库 install.sh，需重启 Harness；write 默认关。MIT，Node>=20。
