dsh-steer-anchor：爲中途插話加上「仍在原任務」的語義框

DSH 中 Ctrl+Enter steering 在模型側等同普通 user 消息，易被當成新任務。dsh-steer-anchor 在 agent/pre-step 爲 step>1 的用戶 steering prepend 固定規則框，引導保持主線並將插話作補充或糾偏；安裝命令爲 dsh plugin --profile add github:yelloooooooow/dsh-steer-anchor，重啓生效。每條約 198 字符，無額外模型調用。MIT，維護者 yelloooooooow。
