dsh-sandbox-noop-escalation：去掉无法放宽沙箱的 sandbox_permissions，避免 GPT 调用被拒

DSH escalating 工具要求 sandbox_permissions 只能等于或宽于当前沙箱模式；GPT 模型常误填 workspace-write，在 danger-full-access 会话下触发 not strictly wider 错误。插件 dsh-sandbox-noop-escalation 在 tools/pre-execute 钩子中移除无法加宽模式的冗余 sandbox_permissions 与 justification，不扩大权限。安装：dsh plugin --profile <name> add dsh-sandbox-noop-escalation。可配置 tools 列表，MIT 许可，适配 DSH 0.2.0-rc.2。
