dsh-model-provider-label: Display provider and model name in the DSH model selector

dsh-model-provider-label is a Web plugin for DeepSeek Harness (DSH) designed to address the difficulty in distinguishing models with identical names when using multiple Providers. The plugin displays both the Provider's display name and the model name in the model selector, helping users accurately identify the source. Developed by haiyoucuv and licensed under the MIT License, it functions by reusing host services and masking default UI slots. Currently, the plugin is no longer maintained, and its core functionality has been integrated into dsh-chat-tweaks. Users should note that these two plugins occupy the same UI slot, and their coexistence will result in errors. Therefore, if dsh-chat-tweaks is installed, dsh-model-provider-label should be uninstalled.
