dsh-insight-dock: Context Anchors and the Insight Dock for the DSH Web GUI

`dsh-insight-dock` is a client plugin for the DSH Web GUI, designed to address the interference caused by irrelevant thoughts generated by an agent while it is focused on a task. It uses a context anchoring mechanism to allow the parking of brief, constrained side observations, ensuring that these thoughts remain useful even after the agent's context is lost. The plugin provides actions such as “Hear me out,” “Close,” and “Close and tell the agent why.” Its features include controlling the prompt strength, concurrency limit, and lifecycle of note generation. Note that notes are stored in memory only; restarting `dsh` clears their content, while settings are persisted.
