dsh-agent-flight-recorder：为 DSH 智能体提供运行时健康观测与循环检测

dsh-agent-flight-recorder 是 MathieuPortmann 维护的 DSH 工作流类插件，基于会话日志做只读观测：在最近 100 次工具调用窗口内检测同工具连击、重复错误与高错误率，提供 runtime_health 工具、Live 告警、每轮摘要及 Runtime Health 面板。不联网、不写盘、不拦截工具。安装命令为 dsh plugin --profile web add dsh-agent-flight-recorder；需 Node ^22.19.0 或 >=24，DSH >=0.2.0-rc.2 且 <0.3.0。配置位于 cordis.patch.yml 的 agent-flight-recorder 行。
