claude2dsh: Migrate Claude Code sessions, skills, and memory to DSH native sessions

`claude2dsh` is an MIT-licensed DSH plugin (version 0.2.0-rc.5 candidate) maintained by kirkchinese. It is used to migrate Claude Code's sessions, skills, global instructions, and project memories into DSH-native continuous sessions, and can also export DSH sessions back to Claude Code JSONL. Core commands include session import (returning counts for categories such as previewed, imported, and skipped), skill and global instruction migration, and converting project memories into DSH skill packs.
