Changelog-Capture: Recording User-Visible Changes as They Occur

`changelog-capture` is a workflow plugin designed to help agent developers avoid the difficulty of reconstructing changelogs at release time. It requires user-facing entries to be recorded as code changes are made, in a structured format (Who/What/How to upgrade) and categorized under Added, Changed, Fixed, or Breaking. At release time, the plugin validates entries against the actual diff to ensure the changelog is accurate and reliable. Running alongside the DSH process, it minimizes the cost of generating release notes by capturing entries immediately while the diff is still at hand, preventing important changes from being overlooked.
