dsh-effort-memory: A DSH Plugin for Memory and Restoration of Reasoning Effort Settings

`dsh-effort-memory` is a Host-only plugin for DeepSeek Harness (DSH) that addresses the loss of reasoning effort levels when switching models. It stores user-configured reasoning effort levels keyed by `(provider, model)` and automatically restores them when the session switches back to that model. The plugin includes Loop-safe and Replay-safe mechanisms and is designed for fault tolerance. Memory data is persisted in the `effort_memory` domain, falling back to an in-memory Map if the storage domain is unavailable. By intervening at the code level, the plugin helps ensure consistency in model settings.
