dsh-fail-logger: Embedding Tool Failures into Skills to Reduce Agent Errors Over Time

dsh-fail-logger is a DSH memory-class plugin maintained by Areium. It automatically listens to session/events to capture failure causes from native tools, PTC run_code, and embedded tool calls.
